Description: HealthCare Partners of Nevada is at present recruited Patient Care Assistant (HCC) right now, this job will be assigned in Nevada. The HCC Patient Care Assistant will provide patient outreach duties by creating and scheduling In-Home or in-Clinic health assessment visits for HCPN HCC Clinician or vendor. T! he incumbent will be responsible for managing and tracking the Clinician or vendor’s daily patient visit schedule and also provide administrative support to the processing and managing of encounters assigned to the HCC Programs Team and to help assist in program outreach support. As a Patient Care Assistant, the incumbent will represent the company's patient care relationships. This position is responsible for assisting physicians with patient care by providing clerical, environment and organizational tasks. The position provides information to the patients in order to fully utilize and benefit from all services offered. It works cooperatively with the care team. The position also provides quality appropriate patient care, utilizing customer service skills throughout the patient care continuum.

MINIMUM JOB QUALIFICATIONS AND REQ! UIREMENTS
High School graduate or equivalent.

Pro! fessional background in the healthcare industry.

Medical Assistant, Certified Nursing Assistant, or other medical training is preferred

Completion with a passing grade from a Medical Assistant program accredited through CAAHEP. CMA (Certified Medical Assistant).

Additional college is a plus.

Minimum of one year experience in a patient care setting providing direct patient care; or telephonic patient care management such as health coaching, care coordination, or other wellness program experience

Excellent computer skills.

Knowledge of medical terminology.

Ability to work independently, with minimal supervision.

Must have excellent organizational skills, problem-solving skills and must be computer literate.

Unrestricted drivers license.

Database and Excel skills.

Spanish-speaking preferred.

Knowledge, Skills and Abilities:
Intermediate Proficiency in Micro! soft software applications: Outlook, Word, Excel, PowerPoint.
Accuracy in managing patient demographics; completeness in tracking all actions in centralized database.
Detail-oriented; able to maintain integrity of data for multiple data-points for each encounter or program assessments.
Ability to update and correct demographic information to allow for optimum patient contacts.
Excellent track record of engaging customers or potential customers which can be applied to patient base. Strong program planning, organization, and interpersonal skills.
Excellent written and verbal communication skills
Self-starter with strong work ethic
Exceptional oral communication skills.

Ability to speak clearly.

Uses tact, discretion and sensitivity in dealing with patients.

Deals courteously and efficiently with physicians, insurance companies, attorney's offices and others.

Written communication skills, such as good basic re! ading, writing, math and spelling skills.

Possesses good typin! g/word processing, filing, reception, and other general office skills.

Maintains CPR certification.

Knowledge of Medical Terminology.

Ability to use a stethoscope.

Knowledge of medications and injection procedures.

Minimum of two years work experience in a medical practice environment or related field is preferred.

Understanding and knowledge of standard concepts, practices, and procedures within the scope of a Medical Assistant.

Has the ability to plan and accomplish goals by utilizing their education, skills and judgment.

Must have user knowledge of technical skills and computers, and/or medical practice software system.

Must know windows and non-windows based systems.

Must have working knowledge of ICD-9 and CPT-4 coding.

Ability to use discretion in handling confidential information.
