Position Summary The Telephonic Case Manager is responsible for facilitating the organization and sequencing of appropriate health care services ensuring cost-effective optimal patient managem! ent. This is a staff position that requires clinical experience, collaboration, and independent judgment. Duties include ongoing objective assessment of the patient, family, and treatment plan so that quality of care is maximized while costs are minimized on a long-term basis. The overall goal of the case manager is to facilitate optimal outcomes for all parties involved through advocacy, objectivity, effective communication, and coordination of appropriate health care services.
This nurse interacts with workers’ compensation clients, providers, and injured workers. Internal client interaction includes Workers’ Compensation Operations team, Finance team, and Information Technology team. Functions and Responsibilities § Maintains confidentiality of patient and client information § Assists with orientation of staff, physician advisors, health services providers and patients to utilization review and case management processes § Reviews and analyzes case referra! l information in consultation with claims representatives, hea! lth services providers, employer and/or agent, family, patient and/or attorney if involved § Assesses the patient’s personal and medical history, current status and treatment plan, and prognosis § Facilitate appropriateness of health care services in accordance with ODG and MDA guidelines. § Identifies potential problems in interdisciplinary collaboration § Identifies a comprehensive care plan, through interdisciplinary collaboration, which identifies short and long-term goals § Coordinates health care services and resources to implement the injured worker’s treatment plan § Conducts ongoing evaluation of the patient’s progress and treatment plan as well as effectiveness and appropriateness of the services provided throughout the entire spectrum of care Abe Ghebrehiwet Senior National Account Executive Nurse Resolutions 888.411.2228 ext 204.
